About The Position

Join the AMAZING Southwest Network Team! As a member of an Assertive Community Treatment (ACT) team, functions as the housing specialist with primary responsibility for the provision of housing support services in accordance with SAMHSA's evidence-based practice of permanent supportive housing. Provides comprehensive assessment of housing, residential and activities of daily living skills and needs, and leads in service planning for adult daily living skills and housing support services. Coordinates, monitors, and provides direct services to members in a community setting. Participates in the development of individualized service plans (ISPs) and provides/coordinates services related to ISPs. Provides after-hours care, on-call coverage, and support to all members being served by the ACT team.

Requirements

  • Master's or bachelor's degree in a field related to behavioral health; or a bachelor's degree in a non-related field with one year of related experience; or an associate degree in any field with one year of related experience; or a high school diploma or equivalency certificate with three years of related experience.
  • Minimum one year working with seriously mentally ill adults required.
  • Previous experience working on an ACT team or equivalent experience preferred.
  • Minimum one year experience working with behavioral health residential programs and/or housing-related services and resources required.
  • Must have and maintain a valid Arizona driver's license.
  • Must be at least 21 years of age.

Responsibilities

  • Function as the housing specialist on the ACT team.
  • Provide housing support services in accordance with SAMHSA's evidence-based practice.
  • Conduct comprehensive assessments of housing and daily living skills.
  • Lead service planning for adult daily living skills and housing support services.
  • Coordinate, monitor, and provide direct services to members in a community setting.
  • Participate in the development of individualized service plans (ISPs).
  • Provide and coordinate services related to ISPs.
  • Provide after-hours care and on-call coverage for members.
