About The Position

Millennium Physician Group is seeking to hire a full-time / Case Manager to join the ACO team. We are looking for a new team member who is positive, energetic, well-organized, can multi-task, and think outside of the box. We want someone who supports our top initiative of ensuring an excellent patient experience. The Case Manager is responsible for the direct provision of services that assist patients with the achievement of their highest level of wellness. The Case Manager focuses on client-centered services that link clients and their family members with health care, and other home or community-based services to ensure timely, coordinated access to medically appropriate levels of health and support services and continuity of care. They are responsible for assessment, planning, coordination, advocacy, education, monitoring, and accurate, timely documentation in all program areas. They demonstrate knowledge of growth and development and possess the ability to assess, interact, communicate, and care for age-specific patient populations.

Requirements

  • Registered Nurse; active and clear license in the State of Florida.
  • 3+ years of clinical experience.
  • 1+ years of case management experience preferred.
  • Strong computer skills, especially in Excel.
  • Knowledge of EMR systems; Athena knowledge preferred but not required.
  • Strong clinical skills.
  • Excellent written and verbal communication skills.

Nice To Haves

  • Experience working in a hospital health system.
  • 2 years or more case management experience.
