About The Position

The ACO Case Manager at Millennium Physician Group is responsible for providing direct services to assist patients in achieving their highest level of wellness. This role focuses on client-centered services, linking patients and their families with healthcare and community-based services to ensure timely access to appropriate levels of care. The Case Manager will engage in assessment, planning, coordination, advocacy, education, monitoring, and documentation, while demonstrating knowledge of growth and development for age-specific patient populations.

Requirements

• Registered Nurse with an active and clear license in the State of Florida.
• 3+ years of clinical experience, with 1+ years of case management experience preferred.
• Strong computer skills, especially in Excel, and knowledge of EMR systems.

Nice To Haves

• Knowledge of Athena EMR system preferred but not required.

Responsibilities

• Provide direct services to assist patients in achieving wellness.
• Link clients and their families with healthcare and community-based services.
• Ensure timely and coordinated access to medically appropriate levels of care.
• Conduct assessments and develop care plans for patients.
• Advocate for patients and educate them about available services.
• Monitor patient progress and document all interactions accurately and timely.
