About The Position

The ACO Program Manager serves as the central lead for Open Door Community Health Centers’ (ODCHC) Accountable Care Organization (ACO) initiatives. This position is responsible for overseeing strategic planning, performance monitoring, and cross-departmental alignment to support successful participation in value-based care models—particularly those focused on Medicare populations. The ACO Program Manager drives performance improvement efforts, supports compliance, and ensures the organization maximizes both quality outcomes and shared savings opportunities. Compensation Range: $95,238.00-$110,595.13 Hmong and Spanish speakers who successfully pass a language exam will receive a minimum of .75 cents added to their wage.

Requirements

  • Strong understanding of Medicare, value-based care, and alternative payment models including ACOs, Medicare Shared Savings Programs, and ACO Convener structures.
  • Experience in healthcare administration, consulting, or value-based care programs, preferably within an FQHC or similar setting.
  • Demonstrated success in leading cross-functional teams, strategic planning, and work plan execution in a healthcare setting.
  • Proven ability to interpret data, assess performance gaps, evaluate quality, identify risks, and develop data-informed solutions.
  • Familiarity with healthcare regulatory compliance, audit processes, and performance monitoring.
  • Strong project management skills and proficiency with tools such as Microsoft Office 365 (Word, Excel, Teams, PowerPoint), Smartsheet, and EHR systems.
  • Excellent written, verbal, and interpersonal communication skills for engaging diverse internal and external stakeholders.
  • Ability to multi-task, think analytically, and adapt in a dynamic environment.
  • Prior supervisory experience and demonstrated leadership ability.
  • Bachelor’s or degree master’s degree preferred
  • Project management experience
  • Requires (3) years of relevant and significant experience in directing and managing large-scale systems projects
  • Requires (5) years of experience in healthcare setting

Responsibilities

  • Lead the implementation, evaluation, and continuous improvement of ACO-related programs and initiatives, including Chronic Care Management and Transitions of Care.
  • Leads and supervises the Care Navigation Specialists to ensure high-quality execution of ACO program activities, supporting care coordination, quality performance, and value-based care goals.
  • Maintain alignment of ACO activities with organizational goals, payer contract expectations, and regulatory compliance.
  • Monitor performance against quality, utilization, and financial targets; develop plans for continuous improvement.
  • Collaborate with the Business Intelligence team to design, maintain, and disseminate actionable dashboards and reports on ACO metrics.
  • Translate complex data into user-friendly insights to drive operational decisions and clinical interventions.
  • Track risk adjustment performance, quality measure adherence, and care gap closure rates.
  • Facilitate workflows between departments including Clinical Operations, Quality Improvement, EHR/Health IT, and Billing to ensure successful ACO performance.
  • Identify and mitigate operational barriers to achieving ACO goals.
  • Serve as the primary liaison between ODCHC and ACO partners, including payers, networks, and regulatory agencies.
  • Coordinate meetings, share updates, and represent ODCHC in external forums related to ACO and value-based care.
  • Identify training needs across departments to support effective documentation, coding (including HCC), and patient engagement in preventive services.
  • Develop resources, job aids, and process guides to support frontline staff and providers in achieving ACO goals.
  • Participate in audits and data validation efforts to ensure accuracy and compliance.
  • Maintain data accuracy within the Aledade App
  • Keep provider roster and location information current
  • Update patient PCP and Location when appropriate
