About The Position

Account Liaison Full Time (80hrs. per biweekly pay period) 8am - 5pm Essential Functions Build and maintain strong, trust-based relationships with a wide range of referral sources, including hospital systems, outpatient providers, senior living communities, home care agencies and nontraditional community partners such as churches, funeral homes, nonprofit organizations and associations. Use data and analytics to understand referral patterns, market needs and program performance, identifying opportunities to grow patient census and optimize outreach efforts. Develop targeted plans based on this analysis, including determining visit frequency and crafting specific messaging to engage referral sources effectively. Gather and share feedback from referral sources with internal teams to help improve processes, service delivery and patient experience. Conduct follow-ups and service evaluations after admissions, discharges or patient deaths, modeling professionalism and ethical conduct. Conduct informational visits at senior living communities, assisted living facilities and personal care homes to educate patients and their families about Choices Healthcare services, with the goal of increasing awareness, generating referrals and promoting the hospice mission. Maintain detailed, accurate and professional documentation of all referral source interactions, educational activities and relationship updates in the organization’s CRM system. Tailor presentations, materials and messaging to meet the unique needs of each referral source, combining business insights with relational engagement to maximize understanding and impact. Partner with internal teams, including clinical staff and the Community Educator, to coordinate educational sessions, facilitate team-to-team meetings, share key information and foster collaboration that strengthens relationships and supports referral growth. Promote and educate patients, families and the community about all Choices Healthcare programs and services, including hospice, palliative care, bereavement support, Inpatient center, Carolyn’s House, educational initiatives and community events. Balance the dual focus of strategic business development and relational excellence to ensure Choices Healthcare continues to grow both numerically and in reputation. As an exempt employee, must be willing to work necessary hours to meet job requirements.

Requirements

  • High school graduate or GED required.
  • Associates degree in marketing, communications or business REQUIRED OR Degree in social work/nursing REQUIRED
  • Minimum of 2 years of sales/marketing experience REQUIRED
  • Must work a minimum of 64 hours per two-week pay period
  • Must work a minimum of 40 hours per two week pay period
  • Must be vaccinated for influenza or be granted a religious/medical exemption.
