About The Position

As a Hospice Care Consultant at AccentCare, you will play a vital role in providing compassionate care and support to patients and their families. This position involves building meaningful relationships with healthcare professionals and collaborating with various teams to ensure that hospice care is delivered effectively and with dignity. You will be responsible for educating referral sources about hospice services, maintaining accurate documentation, and ensuring smooth transitions into care. The role requires a commitment to quality, ethics, and continuous learning in a supportive and purpose-driven environment.

Requirements

  • Bachelor's degree preferred, or 5+ years of experience in hospice, home health, healthcare sales, or marketing in lieu of a degree.
  • Excellent written and verbal communication skills.
  • Strong ability to collaborate with a wide range of stakeholders and interdisciplinary teams.
  • Skilled in problem-solving and understanding referral patterns.
  • Proficient with Microsoft Office Suite.
  • Physically capable of frequent standing, walking, typing, and working on hard/rough surfaces.

Nice To Haves

  • Experience navigating or learning Medicare regulations and hospice reimbursement practices.
  • Availability for after-hours and weekend referrals as needed.

Responsibilities

  • Build meaningful relationships with physicians, nurses, and care facility staff through daily in-person visits and educational conversations.
  • Collaborate closely with Admissions and Marketing teams to develop and carry out community outreach plans.
  • Provide consistent education and compassionate guidance to referral sources.
  • Maintain accurate and timely documentation in the CRM system.
  • Partner with clinical staff to ensure smooth transitions into care.
  • Uphold ongoing commitments to quality, ethics, and learning.
