About The Position

At Prevea Health. the Access One Registered Nurse serves as a critical liaison between referring and accepting facilities, ensuring efficient coordination and communication for patient consults and transfers into HSHS Wisconsin hospitals. This position requires advanced nursing judgment, strong telephonic triage skills, and proficiency in multiple systems to facilitate high-quality, timely patient care. The RN works closely with providers, nursing supervisors, registration staff, and external facilities to ensure smooth patient transitions and adherence to organizational protocols and is also responsible for handling initial triage for Prevea departments outside of regular clinic hours.

Requirements

  • Graduation from an accredited and approved Nursing program Required
  • 1-3 years Registered Nurse experience Required
  • Use nursing assessment and decision-making skills to promote optimal patient outcomes and ensure timely, appropriate care.
  • Skilled in establishing and maintaining effective work relationships with patients, medical staff, and the public
  • Excellent interpersonal skills and behaviors
  • Knowledge of common safety hazards and precautions to establish a safe work environment
  • Competent in patient education/teaching
  • Strong patient assessment skills on tech phone in a high volume, fast-paced environment
  • Current Wisconsin RN license (or compact equivalent; nurses must maintain or update their primary state of residence to Wisconsin to ensure continued eligibility under the compact) Upon Hire Required
  • CPR/BLS - Basic Life Support for Healthcare Providers certification from an approved program Upon Hire Required

Responsibilities

  • Act as the central point of contact for incoming transfers and provider consults; obtain, enter, and verify accurate patient information for registration and tracking.
  • Connect referring and accepting providers using multi-line conference calls, paging systems, and online on-call schedules to ensure efficient patient hand-offs.
  • Work closely with nursing supervisors, charge nurses, and registration staff to determine bed availability and ensure timely assignment of account numbers.
  • Accurately document all interactions and transfer details in Epic, following workflows and protocols for various transfer types.
  • Request and receive medical records and diagnostic images from referring facilities to support care coordination.
  • Assess patient symptoms, provide care advice, initiate protocols, and triage patients to appropriate levels of care using clinical guidelines.
  • Process medication refills per protocol, review EMRs, and relay critical lab results to on-call providers promptly.
  • Provide primary triage services for Prevea departments during non-clinic hours.
