Completes an assessment phone call with each patient and makes a clinical decision about the most appropriate placement and type of appointment. Ensures appointments are scheduled correctly in Epic, provides appointment confirmations, and required paperwork for families. Works closely with clinical and support staff to ensure a smooth and timely process of information gathering, decision-making and scheduling of new patients.
