PURPOSE The Access Center Treatment Coordinator facilitates timely access to behavioral health services by ensuring clients are connected to the appropriate level of care. This role serves as a liaison between medical providers and outpatient services to ensure continuity of care while monitoring treatment progress.

Requirements

  • Bachelor’s degree in behavioral health, social services, healthcare administration, or related field.
  • Minimum of one (1) year of experience in healthcare, behavioral health, or a related setting

Responsibilities

  • Develop initial individualized treatment plans for clients following their first medical visit
  • Complete ongoing treatment plan reviews reflecting client progress
  • Administer and document outcome measures (POMs, FARS/CFARS, FAASM, LOCUS)
  • Maintain accurate and timely documentation in the electronic health record (EHR)
  • Coordinate internal and external referrals based on client needs
  • Obtain required authorizations for release of information
  • Serve as a point of contact for clients, including appointment reminders and coordination
  • Participate in CPI trainings, treatment team meetings, and performance improvement initiatives & perform other duties as assigned
