About The Position

The Case Manager - Residential position involves providing comprehensive case management services to clients transitioning from emergency shelter into short-term housing. This role is crucial in assisting 18-24-year-old transition-aged youth, ensuring they receive the necessary support to achieve self-sufficiency. The case management process includes initial assessments, ongoing evaluations, and regular follow-ups to monitor progress and address any challenges that may arise during the transition period. In this role, the Case Manager will establish individualized service plans for each client, which will include referrals to community-based resources such as Fuel Assistance, Food Stamps, and food pantries. The Case Manager will coordinate all services outlined in the service plan and work collaboratively with other staff members to support clients in their housing and employment searches. Building respectful relationships with clients is essential, as regular home visits will be conducted to monitor adherence to the service plan and provide necessary support. The Case Manager will maintain weekly contact with clients, reassessing their needs and documenting their progress. They will also assist clients in developing positive relationships with landlords, mediating conflicts when necessary, and advocating for clients in Housing Court. In cases where tenancy is at risk, the Case Manager will work to resolve issues to help clients retain their housing or find sustainable alternatives. Additionally, the Case Manager will participate in team meetings, conduct workshops, and assist clients with appointments and transportation arrangements. Confidentiality is paramount, and the Case Manager must ensure compliance with all relevant laws and agency policies. This position requires a proactive approach to engaging clients and addressing their needs, both within and outside the scope of the role.
