Inpatient Pharmacy Intern. Provide clinical and operational pharmacy services to patients admitted to St. John's Hospital. Assist the pharmacy manager in the operation and management of the pharmacy as needed. Provide indirect supervision for support staff and ensure adherence to all policies and procedures.

  • Complete admission medication histories for patients being admitted to St. John's Hospital, with a focus on patients admitted through the Emergency Department.
  • Train and work as inpatient pharmacy technicians and additional roles and responsibilities will be added as the intern progresses in the program.
  • Provide appropriate clinical care including patient counseling and prospective drug use evaluation to all patients receiving their medication therapy from the Pharmacy.
  • Perform initial patient evaluation as it pertains to the prescribed therapies.
  • Collect, analyze, interpret, and evaluate appropriate data relative to medication therapy.
  • Evaluate the significance of signs and symptoms relative to disease processes and make recommendations for the modification of drug therapy.
  • Evaluate the significance and clinical importance of adverse reactions and complications associated with drug therapy.
  • Monitor patient records for potential drug interactions as well as patient allergies.
  • Provide counseling and answer questions from patients, family and health care professionals concerning drug therapy.
  • Provide prescription drug therapy to patients/customers ensuring accurate labeling and dispensing of medications in a timely fashion.
  • Identify and resolve patient specific medication related problems and issues in the patient’s best interest.
  • Ensure dispensing of prescriptions in accordance with all legal requirements, regulatory agencies, third party contracts, and Fairview Pharmacy Services Policy and Procedures.
  • Responsible for error management and medication safety activities.
  • Responsible for accurate and complete billing to payers.
  • Utilize all technologies to ensure accuracy and efficiency in the dispensing process.
  • Participate in team meetings/discussions with clinical pharmacy specialists, pharmacy residents, pharmacy students and technicians.
  • Demonstrates ability to provide care or service adjusting approaches to reflect developmental level and cultural differences of population served.
  • Partners with patient care giver in care/decision making.
  • Communicates in a respective manner.
  • Ensures a safe, secure environment.
  • Individualizes plan of care to meet patient needs.
  • Modifies clinical interventions based on population served.
  • Provides patient education based on assessment of learning needs of patient/care giver.
  • Fulfills all organizational requirements.
  • Completes all required learning relevant to the role.
  • Complies with and maintains knowledge of all relevant laws, regulation, policies, procedures and standards.
  • Fosters a culture of improvement, efficiency and innovative thinking.
  • Performs other duties as assigned.
  • Enrolled as a Student at an ACPE accredited school of Pharmacy beginning Fall.
  • Enrolled as a Student at the University of Minnesota School of Pharmacy.
  • Current Minnesota Pharmacy Intern License.
