About The Position
The position involves providing Case Management supports to a caseload of children or adults with intellectual and developmental disabilities, focusing on facilitating connection and belonging in the community. The right candidate will have a desire to help people author their lives through person-centered planning with the support of family and friends, and the utilization of community and Medicaid supports. Active consultation is provided to those receiving support, including children or adults living with their families, adults in supported living situations, group homes, or living independently in the community. The role requires treating to need and supporting people through a variety of situations and different environments. Additional responsibilities include ongoing consultation with people and their teams, collateral activities with agencies related to supports, coordinating transitions, helping individuals maintain benefits and entitlements, and detailed documentation. The candidate will be responsible for carrying out all activities of the program/sub-unit in a manner that fulfills CMHA-CEI’s mission, policies, and procedures.
