About The Position

Job Overview Provide clinical care to patients of the pharmacy. Assist the Oncology Pharmacists in the management of patients taking oral chemotherapy. Provide appropriate clinical care including patient counseling and prospective drug use evaluation to all patients receiving their medication therapy from the Pharmacy. Perform initial patient evaluation as it pertains to the prescribed oncolytic therapies. Collect, analyze, interpret, and evaluate appropriate data relative to medication therapy. Evaluate the significance of signs and symptoms relative to disease processes and makes recommendations for the modification of drug therapy. Evaluate the significance and clinical importance of adverse reactions and complications associated with drug therapy. Monitor patient records for potential drug interactions as well as patient allergies. Provide counseling and answer questions from patients, family and health care professionals concerning drug therapy. Communicate with patients’ care teams to manage adverse effects, supportive care needs, and changes to oral oncolytic therapy Identify and resolve patient specific medication related problems and issues in the patient’s best interest. Ensure dispensing of prescriptions in accordance with all legal requirements, regulatory agencies, third party contracts, and Fairview Pharmacy Services Policy and Procedures. Utilize all technologies to ensure accuracy and efficiency in the dispensing process. Communicates in a respective manner. Ensures a safe, secure environment. Individualizes plan of care to meet patient needs. Modifies clinical interventions based on population served. Provides patient education based on as assessment of learning needs of patient/care giver. Fulfills all organizational requirements Completes all required learning relevant to the role Complies with and maintains knowledge of all relevant laws, regulation, policies, procedures, and standards. Fosters a culture of improvement, efficiency, and innovative thinking. Performs other duties as assigned Minimum Education: Enrolled as a Student at an ACPE accredited school of Pharmacy beginning Fall. Finished First Year of Pharmacy School Preferred: Enrolled as a Student at the University of Minnesota School of Pharmacy. 2nd year pharmacy student transitioning to 3rd year after this spring.

Requirements

  • Enrolled as a Student at an ACPE accredited school of Pharmacy beginning Fall.
  • Finished First Year of Pharmacy School
  • Current Minnesota Pharmacy Intern License
  • Excellent customer service skills
  • Proficient in English language
  • Capable of solving problems effectively
  • Ability to follow policies and procedures
  • Competent mathematical skills

Nice To Haves

  • Enrolled as a Student at the University of Minnesota School of Pharmacy.
  • 2nd year pharmacy student transitioning to 3rd year after this spring.
  • Previous pharmacy experience preferred.
