About The Position

Adventist HealthCare Home Health is seeking a Speech Language Pathologist (CCC-SLP) who will embrace our mission to extend God's care through the ministry of physical, mental and spiritual healing. As a Speech Language Pathologist (CCC-SLP) you will perform an initial examination and evaluation to establish a impairment/diagnosis, prognosis and plan of care prior to intervention for all persons served with basic rehabilitation needs in acute care, outpatient or inpatient rehabilitation settings. You will provide, or direct and supervise, the therapy intervention consistent with the results of the examination, evaluation, diagnosis, prognosis, and plan of care. The therapist involves the patient/client and appropriate others in the planning, implementation, and assessment of the plan of care. You will also plan for discharge of the patient/client taking into consideration achievement of anticipated goals and expected outcomes, and provide for appropriate follow-up or referral. Communication, coordination, and documentation of all aspects of patient/client management are essential, as well as managing your schedule to accommodate patient load and attending team meetings to communicate patient status.

Requirements

  • Bachelors, Masters, or Doctorate in Speech-language pathology from accredited speech-language pathology program.
  • 1-year home health experience preferred.
  • Valid driver's license with agency insurance requirements.
  • Active American Heart Association Basic Life Support (BLS) certification required.
  • Active Maryland License.

Responsibilities

  • Perform an initial examination and evaluation to establish a impairment/diagnosis, prognosis and plan of care prior to intervention.
  • Provide, or direct and supervise, the therapy intervention consistent with the results of the examination, evaluation, diagnosis, prognosis, and plan of care.
  • Involve the patient/client and appropriate others in the planning, implementation, and assessment of the plan of care.
  • Reexamine the patient/client as necessary during an episode of care to evaluate progress or change in patient/client status.
  • Plan for discharge of the patient/client taking into consideration achievement of anticipated goals and expected outcomes.
  • Communicate, coordinate and document all aspects of patient/client management.
  • Supervise and direct the patient care activities of assigned rehab aides, office assistants and volunteers.
  • Manage schedule to accommodate range in census and patient load.
  • Attend team meetings and effectively communicate patient status.
