Join the University of Chicago Medicine as an Advanced Practice Provider (Nurse Practitioner or Physician Assistant) for Transitional Care Management (TCM) Virtual Visits Team (on-site) within our Care Transitions Clinic. The Advanced Practice Provider will be responsible for comprehensive direct patient care management and will manage a caseload of patients by exercising autonomous decision making in the assessment, diagnosis, and initiation of delegated medical treatments. The APP will provide healthcare and patient services that may include educational, research, and clinical support, as well as complete administrative activities under the direction and supervision of the Director/leadership. The APP has acquired the knowledge, skills, and judgment necessary to assist the physician through organized instruction and supervised practice. The Transition of Care Management (TCM) APP will perform virtual patient care visits (to be done on-site), providing follow up care for adult patients post-hospital discharge. The goal of these visits is to ensure smooth transition back to Primary Care and Specialty providers, helping to minimize ED utilization and re-hospitalization when possible. This position will be housed in our Care Transitions Clinic where these TCM visits are conducted alongside other programs and services aimed at reducing acute care utilization. As such, there may be an opportunity to cross-train with other Care Transitions Clinic programs and services for in-person visits including in-person TCM visits along with other visit types.
