The SSVF Case Manager Lead contributes to HIP's Mission by coordinating and tracking services on the SSVF grant as well as providing client care based on household strengths, identified needs, and barriers towards the goal of sustainable housing and employment or income with solid community connections. Due to the intensive tracking and eligibility requirements for the SSVF grant which includes Shallow Subsidy, HIP is seeking an SSVF Lead Case Manager to track all serves, eligibility, and client assistance offered and allowable to Veterans enrolled. The SSVF Lead Case Manager will be a part of the overall HIP Case Management team but will act as lead in approving all SSVF expenses. The SSVF Case Manager Lead will also assist with Veteran community coordination, Veteran outreach, and claims, reporting, and audits. The SSVF Case Manager Lead assist with professional services in a client-centered atmosphere offering full service and linkage to meet all expressed needs. Key outcomes include: mastering SSVF policy and procedures; tracking eligibility for TFA for each Veteran and approve all TFA expenses; track and ensure all recertifications are completed; support HIP Grant Manager and Veteran Coordinator in providing essential reporting and services to Veterans; building rapport with clients to assess needs and provide support towards mtg those needs; ensuring eligibility for all funding and support recommendations by assisting the household in obtaining necessary documentation; assessing strengths, barriers, needs, and desires towards housing and employment sustainability; identification of and coordination of care both internally at HIP and externally in the community to ensure needs and desires are met; accurate data collection and entry; developing collaborative S.M.A.R.T. goal plans with all essential staff; and supporting clients in all aspects of maintaining housing and employment/income.
