About The Position

Westside Care Center is seeking a temporary part-time Social Worker with experience in long-term care. The role involves coordinating and facilitating social work services for residents, ensuring compliance with state and federal regulations, and providing support to residents and their families.

Requirements

• Bachelor's Degree in Social Work or a related human services field is required.
• A minimum of one year of experience in long-term care providing direct social services is preferred.

Responsibilities

• Conduct face-to-face resident interviews and assessments of cognitive, mood, and behavioral status, along with other psychosocial factors.
• Participate in planning for resident discharges.
• Respond to clinical emergencies, regulatory agency visits, surveys, and issues outside of normal work hours as needed.
• Direct and supervise the delivery of medically related social services to residents and their families.
• Participate in the clinical evaluation and screening of potential admissions to the facility as needed.
• Collaborate with the interdisciplinary treatment team to access information and make referrals to community-based services for residents.
• Serve as a liaison with facility contracted mental health practitioners and consultants.
• Develop and implement new behavioral and psychosocial service programs and present them to physicians, family members, and residents.
