About The Position

Join the St. Cloud Hospital inpatient care management team as a float Social Worker. This role has the ability to float to almost all of the units at St. Cloud Hospital to provide social work support. Our Social Workers at the St. Cloud Hospital serve patients and families in our community and beyond by helping them during a challenging time in their lives. Our Social Workers enjoy the variety of cases they are presented with as there is always something new and different. You'll collaborate with other Social Workers to gain knowledge and experience and work with an interdisciplinary team of nurses, therapists, physicians, and dieticians. You will have a designated patient caseload and will plan care to effectively meet the patient/family transitional needs including monitoring the length of stay and promoting community resources. We will provide supervision for new grads in-house or will reimburse up to $175/month for external supervision.

Requirements

• Current Licensed Social Worker (LSW) in Minnesota required.
• Bachelor's degree in social work or Sociology.
• At least two years of experience as a Social Worker, preferably in a healthcare setting.
• American Case Management Association (ACMA) Case Manager-SW Certification required within five years.

Responsibilities

• Provide social work support across various units at St. Cloud Hospital.
• Serve patients and families during challenging times.
• Collaborate with other Social Workers and interdisciplinary teams.

Benefits

• Starting pay begins at $27.95 per hour and increases with experience.
• Pay range: $26.59 - $39.88 per hour.
