About The Position

The RN/LPN Unit Manager at St. Monica Center for Rehab and Healthcare is responsible for overseeing a specific unit within the facility. This role includes managing patient care, medication administration, clinical documentation, and supervising nursing assistants. The Unit Manager ensures compliance with healthcare standards and promotes resident involvement in their care while collaborating with multidisciplinary teams.

Requirements

  • Graduated from an accredited school of nursing and licensed as an LPN/RN in the state of MD.
  • Current CPR certification or certification within 90 days of hire.
  • Mid-level experience as an LPN.

Responsibilities

  • Confirm resident/guest identity prior to medication administration and treatments using dual identifiers.
  • Practice SBAR communication when interacting with healthcare community members.
  • Encourage resident/guest involvement in their own care.
  • Receive detailed reports from outgoing nurses for proper communication and follow-up.
  • Recognize and respond to changes in resident's condition and document appropriately.
  • Initiate and update Plan of Care upon admission as needed.
  • Interact with multidisciplinary service teams to assure continuum of care.
  • Practice Standard Precautions and adhere to infection prevention strategies.
  • Demonstrate compliance with Resident's Rights.
  • Transcribe physician orders correctly on MAR & TAR.
  • Fax POS to Pharmacy provider for timely medication delivery.
  • Maintain a safe environment for residents/guests and report unsafe situations.
  • Follow chain of command in reporting and communication.
  • Adhere to proper hand washing techniques.
  • Administer medications according to policy and procedure.
  • Notify responsible parties of changes in patient's condition.
  • Work closely with Social Workers to resolve residents'/guests' needs.
  • Check syringes and count narcotics at the end of the shift.
  • Order stock and prescription medications as needed.
  • Admit and discharge residents/guests according to facility procedures.
  • Review reports with CNAs prior to their assignment activation.
  • Perform various assessments related to skin, cardiac, respiratory, GI, neurological, muscular skeletal, renal, and pain.
  • Accurately document on various nursing forms and assessments upon admission.

  • Full-time position with competitive salary range of $91.5K - $116K per year.
  • Monday to Friday schedule with occasional on-call weekends.
