About The Position

This position is in the Endocrinology Physician's Office, Monday-Friday, 8a-4:30p, no holidays, no weekends. This individual will be primarily responsible to obtain prior authorizations for medications and procedures, work with patients and providers to schedule procedures, assist provider during office based procedures, and manage phone triage for the office. General Summary: A nonexempt position responsible for performing a variety of duties including, but not limited to coordinating care within the multidisciplinary team through telephone and written (including Patient Portal messaging) interaction with patients to ensure the timely receipt of appropriate medical care. The Triage Nurse acts as a liaison between patients, doctors and related staff to ensure that identified problems and concerns are addressed. The Triage Nurse utilizes nursing knowledge, internal reports, insurance data and manual data entry to record, document and process all transactions. The Triage Nurse operates under the Indiana Nurse Practice Act and directs patients to the right level of care with the right provider in the right place at the right time.

Requirements

  • Registered Nurse from accredited school
  • Must possess active license to practice in the state of Indiana
  • Two years of recent experience working in a medical facility as a registered nurse
  • Must be proficient in computer skills, including Microsoft Office (specifically Word and Excel)
  • Self-disciplined, energetic and passionate
  • Highly organized with well-developed oral and written communication skills
  • Demonstrate sound judgment, decision-making and problem-solving skills

Responsibilities

  • Assists all patients through the healthcare system by acting as a patient advocate and navigator
  • Making outbound and receiving incoming calls to assess patient's current health status
  • Identifying gaps or barriers in treatment plans
  • Interacting with clinical manager on challenging cases
  • Participates in Patient Centered Medical Home meetings and quality improvement meetings monthly
  • Supports patient self-management of disease and behavior modification interventions
  • Manages high risk patient care, including management of patients with multiple co-morbidities or high risk for readmission to a hospital setting, including a registry
  • Promotes clear communication amongst care teams by ensuring awareness regarding patient care plans
  • Documenting and tracking findings
