About The Position

The RN Case Manager at San Jose State University's Student Wellness Center plays a crucial role in supporting students' health and well-being by coordinating care across various services. Requirements

• High School diploma or equivalent
• Completion of a professional nursing program in an approved school of nursing
• Possess and maintain a valid license as a registered nurse in California and certification in CPR/AED
• Three (3) years of progressively responsible full-time professional nursing experience

Nice To Haves

• One (1) year of experience providing case management in a nursing capacity
• Demonstrated experience assisting patients navigating referrals and/or insurance resources
• Demonstrated experience working within an integrated health facility

Responsibilities

• Develop and implement systems for follow-up with students to ensure continuity of care when utilizing multiple Student Wellness Center services
• Support students accessing SWC services for a variety of care needs including mental health, eating disorders, sexual health needs, and gender affirming care
• Assess and triage students referred to Health Services by other SJSU departments to educate on availability of services
• Work with administrative support staff to ensure coordinated care across the SWC, including scheduling appointments and referral follow-through
• Assess appropriateness of external referrals based on a student's insurance and care provider recommendations
• Collaborate with providers on determining appropriateness of other resources when SWC internal resources are not available
• Make appropriate entries in patients' electronic medical records
• Administer immunizations, TB tests, and certain medications under standardized procedures
• Build relationships with campus and community agencies to increase the referral network for students
