About The Position

Community Care is hiring a RN Care Manager to join our Family Care Program team. This is a full-time opportunity working out of our Dane County location. *$8,000 Sign On Bonus

Requirements

• Education: Associates degree in Nursing Required, Bachelors Degree preferred.
• Licensure: Wisconsin Licensed Registered Nurse.
• Licensure: Eligibility for long-term care functional screener certification is required.
• Licensure: WI Drivers License.
• Experience: Required 2 years recent experience with older adults or persons with physical or intellectual disabilities in a nursing capacity Required.
• Preferred 2 years of work in community nursing.

Responsibilities

• Completes discipline-specific initial and ongoing assessments.
• Conducts home visits and home assessments as needed, and consults with families and other supports for additional information as needed.
• Develops and maintains care plans, including members' long-term care and personal outcomes, progress towards past identified outcomes, member strengths, risks, natural and formal supports.
• Follows up on all services to ensure quality/coordination and effectiveness of services.
