About The Position

The Remote Case Manager position is designed for licensed healthcare professionals, specifically Registered Nurses (RNs), Licensed Clinical Social Workers (LCSWs), or Licensed Clinical Professional Counselors (LCPCs), who will conduct medical management and health education programs for clients enrolled in government healthcare programs. The role involves data analysis for regulatory reports and may require home, hospital, or long-term care visits to members.

Requirements

• Must be a licensed RN, LCSW, LCPC, or LCSW residing in Illinois.
• Three years of direct clinical care experience in a clinical setting for RNs or two years of clinical practice for LPCs or LMSWs.
• Current, valid, unrestricted license in the state of operations (or reciprocity).
• Three years of wellness or managed care experience presenting clinical issues with members/physicians.
• Current Illinois driver's license, transportation, and applicable insurance.

Nice To Haves

• Bilingual skills are a plus but not required.

Responsibilities
