About The Position

On Lok PACE is a non-profit committed to the care of elders, focusing on providing comprehensive services to frail seniors. The Home Care Registered Nurse will work under the direction of the Home Care Manager, offering direct skilled care and supervising care for participants receiving home care services. The role emphasizes the importance of community-based care for seniors with chronic needs, ensuring they receive necessary medical and social support.

Requirements

• Graduate of an accredited college or university with either an associate or baccalaureate degree in nursing.
• Possess a current California Registered Nurse license.
• A California Public Health Nurse certificate is desired.
• A minimum of one year of nursing experience is required.
• One-year experience working with the elderly or frail population required.
• Experience in public health nursing is desired.
• Ability to work as part of a multidisciplinary team.
• CA Driver's license and willingness to drive and travel.
• CPR certificate.

Responsibilities

• Evaluates, implements, and develops a home care treatment plan for participants.
• Monitors participant changes and adjusts treatment plans accordingly.
• Provides skilled nursing care as needed.
• Instructs and supervises geriatric aides and LVNs in carrying out the home care treatment plan.
• Teaches and counsels participants and family members regarding participant's care and safety.
• Participates actively in the interdisciplinary team by offering clinical expertise in team planning and problem solving.
• Communicates effectively with participants, family, and team members.
• Performs other related duties as assigned.
