Devoted Medical is the affiliated care delivery partner of Devoted Health. We are hiring a Physician to lead our Complex Care Program, dedicating a portion of your time to direct patient care while guiding interdisciplinary teams. You'll serve as the clinical cornerstone—conducting comprehensive assessments, developing care strategies, and being present at critical moments including patient onboarding and transitions from hospital care. As both clinician and leader, you'll be the cultural beacon for the team, and maintain accountability for medical outcomes across your patient panels. This will require analyzing performance data and driving key metrics. Requirements

• Board Certified Geriatrician, Primary Care, Internal Medicine, or Family Medicine Physician with at least 2 years clinical experience.
• Experience providing clinical care in collaboration with interdisciplinary teams.
• Team player, continuous improvement mentality and can-do attitude.

Nice To Haves

• Experience serving diverse patient populations, with a focus on delivering care that aligns with our mission.
• Experience with virtual care is a plus but not required.

Responsibilities

• Deliver exceptional care to a cohort of complex patients across multiple geographies, leveraging video telehealth visits and an interdisciplinary team consisting of nurse case managers, and field-based team members.
• Clinically manage the patients, with a strong focus on complex geriatric conditions (utilizing the 5M framework) such as polypharmacy, falls and dementia.
• Responsible for clinical performance of their teams by monitoring and trending outcomes, and working with other leaders to ensure teams are meeting performance metrics.
• Assess opportunities to iteratively improve the clinical model, and be a change agent to help advance the clinical model.
• Engage patients, patients' caregivers/family, existing provider network & other Devoted programs to provide cohesive, comprehensive care experiences & optimal clinical outcomes.
• Utilize our home grown electronic health information system (Orinoco) for visits while also providing feedback on how to improve the interface.
• Steward our patient-centric and open clinical culture to our patients and colleagues.
