The Transitions of Care pharmacy technician position has a primary responsibility of performing medication histories with patients in the Emergency Department. The process includes interviewing patients and family members and utilizing online prescription resources to compile the best possible medication list for patients being admitted to the hospital. Excellent communication and problem-solving skills as well as prior hospital pharmacy experience are keys to success with this position. Previous Epic use is also beneficial.

Requirements

• High school diploma or equivalent required.
• Minimum 2 years of experience as a pharmacy technician required; retail and/or inpatient hospital experience preferred.
• Experience applying customer service behaviors and communication skills required.
• Basic familiarity with computers and keyboard skills and ability to navigate electronic systems applicable to job functions.
• Current applicable state Pharmacy Technician Licensure OR Pharmacy Intern License required prior to date of hire and maintained thereafter.

Nice To Haves

• Graduate of a pharmacy technician program preferred.
• Previous Epic use is also beneficial.

Responsibilities

• Gathering the best possible medication history that is then utilized by the care team for medication reconciliation.
• Facilitating the provision of medications at the time of discharge from the hospital.
