About The Position

We are seeking an experienced Occupational Therapist to join our team in providing high-quality patient care. The ideal candidate will possess strong clinical skills and a passion for helping individuals achieve their full potential. The Occupational Therapist will work in a home setting, utilizing their expertise in acute care, occupational health, and patient service to deliver effective treatment plans and support case management. The successful applicant will be responsible for conducting evaluations, developing treatment plans, and collaborating with patients, families, and healthcare professionals to promote optimal health and well-being. We offer not only geriatric care, but we also offer Hospice, Palliative, discharge to home, Alzheimer’s and Dementia care, and concierge medical care.

Requirements

  • A Master's or Doctoral degree in Occupational Therapy from an accredited institution.
  • Current licensure as an Occupational Therapist in Texas.
  • Experience working in a hospital, clinic, or other healthcare setting.
  • Strong knowledge of medical terminology and ability to communicate effectively with patients and healthcare professionals.
  • Experience with case management and patient service.
  • Experience working with patients with a range of diagnoses, including those requiring critical care.
  • A commitment to ongoing professional development and a desire to stay current with best practices in occupational therapy.

Responsibilities

  • Conduct comprehensive assessments of patients' physical, cognitive, and emotional abilities to identify areas of strength and weakness.
  • Develop and implement individualized treatment plans that incorporate speech therapy, physical therapy, and other modalities to improve patients' functional abilities and independence.
  • Utilize medical terminology to accurately document patient progress, treatment plans, and outcomes.
  • Collaborate with patients, families, and healthcare professionals to develop and implement treatment plans that address patients' unique needs and goals.
  • Provide patient education and support to help patients and families navigate the healthcare system and manage their condition.
  • Assist with case management by communicating with healthcare providers, social workers, and other stakeholders to ensure patients receive comprehensive care.
  • Demonstrate a commitment to critical care and patient safety by adhering to hospital or clinic protocols and standards.
  • Maintain a professional demeanor and contribute to a positive work environment by being respectful, empathetic, and supportive of colleagues and patients.
