About The Position

The Nurse Coordinator at the Pearl Barlow Center for Memory Evaluation & Treatment is a patient-focused role responsible for high-quality communication and care coordination for patients. Requirements

  • Licensure as a Professional Registered Nurse in NYS with current registration.
  • BSN or graduate of an accredited RN program with a bachelor's degree in a related healthcare field.
  • Relevant clinical experience in ambulatory care, managed care, or acute medical surgical experience.
  • Demonstrated strengths in interpersonal communication, customer service, negotiation, conflict management, and clinical leadership.
  • Working knowledge of Microsoft Office and proficiency in managing an electronic medical record such as Epic.

Nice To Haves

  • Two or more years in an ambulatory health plan or home health setting or ambulatory setting care/case management experience.
  • Membership in care/case management and/or related professional organization.
  • Certification in Case Management.
  • Epic proficiency.
  • Masters of Science in Nursing or relevant health-related field.

Responsibilities

  • Partner with practice clinicians and staff to ensure high-quality patient communication and consistent patient satisfaction.
  • Contact patients interested in infusions to discuss medication administration protocols, risks, and benefits.
  • Conduct chart reviews for patients pursuing Leqembi infusion.
  • Track the infusion timeline for patients currently receiving Leqembi Infusion.
  • Assist physicians with consenting appointments and answer questions regarding logistics/protocols.
  • Act as a professional role model by considering the needs and behaviors of specific patients in a culturally competent manner.
  • Provide for the educational needs of self, patients, and administrative staff, and participate in developing patient/family education materials.
  • Participate in quality improvement programs that promote satisfaction for patients, families, caregivers, physicians, and staff.
  • Maintain current clinical knowledge and competence to fulfill responsibilities.
  • Interact with clinical and administrative positions in various roles.
  • Adhere to department and FGP policies and procedures.
  • Document communication and care coordination activities in the Epic electronic medical record.
  • Organize and present pertinent data for medical diagnosis and treatment.
  • Coordinate practice activities to provide patient care in a healing environment.
  • Perform other duties as assigned.
