About The Position

The Master Social Worker (MSW) provides psychosocial services to patients treated by the facility, including in-center and home dialysis patients. This role utilizes Social Work Theory of Human Behavior and accepted methods of social work practice to promote positive adjustment, rehabilitation, and improved quality of life for patients. The MSW works collaboratively with the interdisciplinary team to inform, educate, and support staff in understanding the emotional, psychological, and behavioral impacts of Chronic Kidney Disease on patients and their families. This entry-level position supports the Fresenius Kidney Care (FKC) commitment to quality indicators and outcomes, actively participating in process improvement activities that enhance patient satisfaction and quality of life.

Requirements

  • Masters in Social Work.
  • Must have state-required license.
  • Meets applicable scope of practice board and licensure requirements in the state of employment.
  • 0 - 2 years' related experience.

Responsibilities

  • Assess patients' psychosocial status, strengths, and areas of need as part of the comprehensive patient assessment.
  • Participate in care planning in collaboration with the patient and healthcare team to identify effective interventions.
  • Utilize FKC patient education programs and established social work methods to address barriers and meet treatment goals.
  • Provide monitoring and interventions for patients to adjust to dialysis and achieve optimal psychosocial status.
  • Offer supportive counseling services to patients within the scope of clinical training and state license.
  • Assist patients seeking transplant with educational and goal-directed counseling.
  • Provide information and assist with referrals to community resources to facilitate optimal treatment outcomes.
  • Maintain current knowledge of local vocational/educational rehabilitation programs.
  • Assess patient awareness of advance directives and facilitate discussions with healthcare teams and families.
  • Document interactions and interventions provided to patients and/or families.
  • Participate in monthly quality review meetings and report on quality indicators related to adherence.
  • Assess patient knowledge of kidney disease and provide tailored education.
  • Facilitate the transplant referral process and collaborate on transplant waitlist management.
  • Review patient rights and responsibilities with patients and/or their representatives.
  • Support the clinic in patient scheduling to maximize adherence and adjustment.
  • Collaborate with Insurance Coordinators to address insurance-related issues.
  • Provide information and education about payment options for dialysis.
  • Refer patients to the patient billing solutions department for treatment-related billing questions.
  • Assist with the interview process for new personnel if requested.
  • Maintain updated patient resource lists and provide training to staff on psychosocial topics.
  • Participate in weekly team huddles and discuss urgent patient issues.
