About The Position

The LVN for Memory Care/Assisted Living position at Town and Country Manor is a part-time role focused on providing compassionate care to seniors with memory impairment. Requirements

• Relevant experience required
• LVN/LPN License required
• Desire and ability to work with seniors with memory impairment
• Ability to lift up to 50 lbs.
• Ability to bend, reach above head level, sit, and perform repetitive movements

Nice To Haves

• Assisted Living & Memory Care experience (1 year preferred)
• Dementia care experience (1 year preferred)

Responsibilities

• Keep charts up-to-date
• Educate residents regarding health care issues, including new medications
• Administer flu shots annually
• Supervise the performance of caregivers and communicate with the Assisted Living Coordinator or Health & Wellness Director regarding issues
• Process recaps and transfer current orders to new month binders
• Assess resident's need for transfer to another level of care
• Conduct pharmacy audits and follow up on recommendations from pharmacists
• Communicate with physicians regarding changes in condition, new orders, or incidents
• Process assessments, physician's reports, and care plans for each resident
• Manage medication as ordered
• Assess for pain, depression, anxiety, and Chaplaincy needs
