About The Position

Provides a wide range of clinical social work services in a primary care setting at Mattawa Community Medical Clinic, including support and consultation to the care team in managing patients. Evaluate psychosocial assets and deficits for the purpose of enabling the patient to maintain the greatest level of independence. Coordinates with others to establish a plan of care. Provides short-term counseling, teaching, and brokering with other resources for caregiving needs.

Requirements

  • Current and valid state licensure as a Social Worker (LICSW)
  • Minimum of one (1) year of experience in a social work role, preferably in a community center or geriatric setting
  • Thorough knowledge of social work principles, aging-related issues, and available community resources
  • Excellent assessment, counseling, and problem-solving skills
  • Ability to empathize, show compassion, and establish rapport with patients and their families.
  • Strong communication and interpersonal skills, enabling effective collaboration with interdisciplinary teams.
  • Proficiency in using electronic health record (EHR) systems and computer applications.

Responsibilities

  • Conduct thorough assessments of psychosocial needs, considering emotional, mental, and social factors.
  • Collaborate with patients and their families to develop personalized care plans addressing their specific goals and preferences.
  • Offer counseling and emotional support, helping patients navigate life transitions and challenges.
  • Advocate for patients' rights and access to community resources, ensuring they receive appropriate support and services.
  • Work alongside interdisciplinary teams, including nurses, caregivers, and healthcare professionals, to coordinate and enhance care.
  • Facilitate communication and assist in resolving conflicts among family members involved in the patient's care.
  • Educate patients about available community resources, support groups, and programs to enhance their social engagement and overall quality of life.
  • Maintain accurate and up-to-date documentation of patient assessments, care plans, progress notes, and relevant interactions.
  • Stay informed about current social work practices, policies, and regulations, ensuring compliance with ethical standards.
