About The Position

Be a part of a world-class academic healthcare system at Ingalls Memorial Hospital as a Medical Social Worker. The Medical Social Worker provides clinical social work services through psychosocial assessments, treatment (counseling/social treatment interventions), and discharge planning services to patients and their families. Coordinates multi-disciplinary discharge planning process.

Requirements

• Requires a current Illinois license as a Clinical Social Worker (LSW) or eligible.
• Bachelor's degree in social work from an accredited school of social work.
• Minimum of one year's experience as a Social Worker.
• Excellent verbal and written communication and organization skills.
• Must obtain current and active BLS through the American Heart Association.

Responsibilities

• Intervenes with patients who have complex social needs.
• Assists with eligibility determination for social service programs and funding sources.
• Qualifies patients for community assistance from a variety of special funds and agencies.
• Offers crisis intervention to patients and families with psycho-social needs.
• Receives referrals from individual care managers and inter-disciplinary team members.
• Follows and actively supports the Philosophy, Mission, and Vision of Ingalls Hospital.
• Adheres to all hospital policies, procedures, and standards of care.
• Observes and adheres to all corporate compliance requirements.
