About The Position

The Hospice Registered Nurse (RN) position at SouthernCare involves providing compassionate and dignified care to hospice patients and their families. The role focuses on managing patient care, collaborating with an interdisciplinary team, and ensuring that patients receive the necessary support during critical times. The RN will serve as a primary liaison between the patient, family, and healthcare team, emphasizing personal care and comfort in the patient's home environment.

Requirements

  • Compassionate, kind, and empathetic nature towards patients and their families.
  • Comfortable providing care in various home settings.
  • Demonstrate flexibility and composure in crisis situations.
  • RN license in the state of requested employment (or eligibility to obtain).
  • 1+ years of nursing experience in hospice, home health, ICU, geriatric, or oncology preferred.
  • Current driver's license and automobile insurance.
  • Current CPR certification.

Nice To Haves

  • Experience as a hospital Staff RN, Med Surg, PACU, Float pool, ER, critical care, acute care, cardiac, geriatric, admissions, wound care, telemetry, or travel nursing.

Responsibilities

  • Report directly to the Administrator, Executive Director, or Patient Care Manager Senior and maintain primary control and professional management of each patient.
  • Act as primary liaison between physician, patient/family, and hospice team.
  • Provide direct patient care services to hospice patients and supportive care to the patient's family.
  • Empower patient's caregivers by educating them on care for their loved one.
  • Manage and collaborate with an interdisciplinary group consisting of LPNs, CNAs, and support staff.
  • Evaluate patient and family needs and assist in the preparation and execution of the plan of care for each patient.
  • Maintain all assigned patient clinical records.
  • Participate in company quality assessment and performance improvement programs.
