About The Position

This position involves collaborating with interdisciplinary teams to deliver quality patient care under the direction of a registered nurse. The role includes assisting in performing personal care needs and providing compassionate end-of-life care. The position will float to four hospice home locations and requires 12-hour shifts from 7 PM to 7:30 AM, with weekend coverage as needed.

Requirements

• California Home Health Aide (HHA) - CA Department of Public Health - REQUIRED
• California Certified Nurse Assistant (CNA) - CA Department of Public Health - REQUIRED
• AHA Basic Life Support for Healthcare Professional (AHA BLS Healthcare) - American Heart Association - REQUIRED
• Driver's License - CA Department of Motor Vehicles - REQUIRED
• ServSafe Food Handler Certified - National Restaurant Association - REQUIRED

Nice To Haves

• 1 Year experience in acute care, home health, or hospice.
• Current Food Handler card or acquisition within 90 days of hire.

Responsibilities

• Collaborate with interdisciplinary teams in the delivery of quality patient care under the direction of a registered nurse.
• Assist in performing personal care needs and compassionate end-of-life care.
• Contribute to overall unit functions.
• Observe patient condition and report changes to the RN.
• Communicate and participate in the coordination of the plan of care to meet established goals.
• Perform routine equipment maintenance according to departmental policy.
• Maintain all certifications required of the job.
• Provide comprehensive personal care to patients under the supervision of the RN.
• Demonstrate effective communication to patients/families in alignment with hospice philosophy.
• Maintain a safe working environment for patients, families, and team members.

Benefits

• Hourly Pay Range: $24.000 - $26.770 - $29.530
• Covered by a Collective Bargaining Agreement (CBA) with SEIU-UHW.
