About The Position

The Exercise Physiologist in Cardiac Rehab at Advocate Lutheran General Hospital is responsible for assessing patients systematically to ensure safe and effective exercise. This role involves interviewing patients to obtain their history, documenting assessments, and identifying desired outcomes related to exercise and cardiac risk reduction. The physiologist will also develop individualized exercise plans, conduct exercise sessions, and provide education to patients and the community regarding cardiac conditioning. The position requires participation in quality improvement projects and adherence to AACVPR accreditation standards.

Requirements

  • Masters Degree in Exercise Physiology/Cardiac Rehab
  • Clinically appropriate skills
  • Ability to work cooperatively with internal and external customers
  • Proficient in telemetry monitoring and arrhythmia interpretation
  • Computer skills
  • Able to teach classes to 15-20 people
  • ACLS Certification
  • BLS Certification

Responsibilities

  • Demonstrates the ability to appropriately assess the patient in a systematic way to assure safe and effective exercise; documents patient assessment and teaching.
  • Interviews and obtains patient history, utilizing direct and indirect sources of data.
  • Documents assessments appropriately.
  • Identifies desired outcomes with the patient related to exercise and cardiac risk reduction.
  • Identifies learning needs of patient and family and incorporates into the educational goals for the patient.
  • Initiates referrals to the health care team.
  • Anticipates and identifies changes in patient condition and takes appropriate action.
  • Assists in the referral process between Phase I, Phase II, and Phase III Cardiac Rehab.
  • Develops an individualized exercise plan and establishes priorities for the patient based upon exercise prescription from the physician.
  • Involves patient and family in the development of the exercise plan.
  • Identifies need for life-long commitment to an exercise program and cardiac risk reduction plan for patients.
  • Utilizes telemetry monitoring, vital signs, and symptom observation before, during, and after exercise, reporting significant changes appropriately.
  • Selects appropriate psychosocial interventions based upon age and individual requirements.
  • Implements patient education plans based upon assessment.
  • Provides patient and community education regarding cardiac conditioning.
  • Provides a safe environment and instructs patients on appropriate use of exercise equipment.
  • Evaluates plan of care against identified patient outcomes and reports relevant changes to prescribing physicians.
  • Participates in quality efforts through department based on quality improvement projects.
  • Actively participates in patient rounding with the medical director to meet CMS and program requirements.
  • Accepts professional responsibility by participating in activities which further professional growth and are beneficial to the department and the patients we serve.
  • Incorporates information and skills acquired in learning activities to improve care to the patient; shares information with fellow staff members.
  • Accepts leadership responsibilities and functions in the charge role when assigned.
  • Mentors new staff and interns as applicable.
  • Maintains confidentiality and respects patients' right to privacy.
  • Provides care while respecting rights of patients and families.
  • Actively participates in activities required to meet AACVPR accreditation standards.
  • Participates in research, development and implementation of new programs related to cardiac risk reduction.
