About The Position

The Engagement Specialist at New Vista of the Bluegrass Inc. serves as a central wellness point of contact for clients within the healthcare system. This role focuses on coordinating care among team members and linking clients to health and wellness services, ensuring that the client's voice is prioritized. The position requires a proactive approach to learning about healthcare services and aims to provide a universal gateway for clients to access various New Vista services, promoting a culture of inclusion and teamwork.

Requirements

  • High School diploma or equivalent.
  • Minimum of 6 months experience in healthcare or customer service.
  • Valid driver's license and proof of auto insurance.

Nice To Haves

  • Experience providing case management services.
  • CMA or bachelor's degree in Psychology, Social Work, Public Health, or related field.
  • Experience in integrated care service (both behavioral and physical healthcare).

Responsibilities

  • Ensure clients receive services in a timely and effective manner, identifying and removing barriers to access.
  • Facilitate communication between clients, family members, and the care team to ensure client satisfaction and quality of care.
  • Evaluate social determinants of health and link clients to resources such as housing supports, food, and financial assistance.
  • Intervene when appointments are missed to determine reasons and barriers to access.
  • Identify and link clients to proper services for those with limited English proficiency or other special needs.
  • Ensure continuity of care between internal and external providers and support transitions or adjustments to care.
  • Advocate with providers and payors for proper healthcare access.
  • Determine creative solutions to barriers to care through connections to resources.
  • Become familiar with local medical providers and how to access benefits.
  • Track appointments, medication, and wellness needs for clients on caseload.
  • Connect with the care team to ensure clients do not fall through the cracks using creativity and teamwork.
  • Cross-train with other aspects of the Call Center, including the Helpline and National Suicide Prevention Lifeline.
