Requirements

Responsibilities

  • Provide guidance to individuals affected by cancer throughout their treatment journey.
  • Assist with resource navigation services for low-income families impacted by cancer.
  • Conduct outreach in designated communities to educate about the Empower Program and recruit participants.
  • Maintain an active caseload of approximately 30 clients, ensuring they receive necessary support and resources.
  • Identify and link participants to resources addressing barriers to care, such as food insecurity and transportation.
  • Distribute cancer prevention and educational materials at community events and local organizations.
  • Document and report on client interactions and resource utilization as required.
  • Ensure compliance with confidentiality regulations regarding Personally Identifiable Information (PII) and Protected Health Information (PHI).
  • Promote cultural awareness and sensitivity within the community served.
