About The Position
The Care Manager, RN at Valley Health Partners is responsible for conducting assessments of patient and family needs, particularly for high-risk patients or those at risk for poor outcomes. This role involves collaboration with health professionals across the continuum of care and the formulation of a documented plan that ensures continuity and goal-setting. The aim is to equip individuals and their families to easily access resources and adopt healthy lifestyles. This position is supported through grant funding and will be subject to reapproval every two years.
