About The Position

Join our friendly, professional, and supportive organization where you can make a difference in someone's life every day! We are looking for dedicated individuals to provide direct adult rehabilitative support services in a community-based transitional housing residence. This role involves working in semi-supervised apartments and requires a commitment to helping consumers achieve their rehabilitation goals. You will be responsible for on-site supervision, counseling, advocacy, and assisting consumers with various daily living activities.

Requirements

• Bachelor's degree in Mental Health or a related discipline; OR Licensed R.N.; OR two years of college plus two years of related work or life experience; OR licensed as a Practical Nurse (LPN) plus two years of related mental health experience; OR a High School diploma and four years of related mental health experience.
• Must possess a valid driver's license with a good driving record.

• Bilingual (English/Spanish) preferred, but not required.

Responsibilities

• Provides on-site supervision, counseling, and advocacy to consumers.
• Assists consumers in achieving their goals as outlined in their individual rehabilitation plan.
• Builds consumer skills utilizing evidence-based practices for activities of daily living.
• Provides assistance with personal hygiene, household chores, meal planning, and shopping.
• Helps with money management, community involvement, and recreational skills.
• Assists with medical case management and employment-related activities.
• Provides transportation in the agency vehicle safely and responsibly.
• Conducts crisis intervention and medication supervision as per agency guidelines.
• Maintains life safety documentation and standards, reporting any facility issues for repair.
• Ensures the environment is clean, safe, and secure at all times.
