About The Position

The Behavioral Health Coordinator at Morris Hospital plays a crucial role in the collaborative care team, working alongside medical providers and psychiatric consultants to deliver integrated mental health care within a primary care setting. This position involves supporting and coordinating the mental and physical health care of patients, ensuring they receive comprehensive support tailored to their needs.

Requirements

• Master's Degree in Social Work or related field from an accredited college or university.
• 3-5 years of experience in a clinical or community health setting.
• State of IL license as a Licensed Clinical Social Worker (LCSW) or Licensed Clinical Professional Counselor (LCPC).
• Experience in Care Coordination and/or Case Management is preferred.
• Working knowledge of differential diagnosis of common mental and/or substance abuse disorders.

Nice To Haves

• Ability to speak a relevant second language.
• Previous experience with health IT systems and data reports.
• Experience with mobilizing community resources and working with diverse populations.

Responsibilities
