About The Position

The Admissions Coordinator at Acacia Network is responsible for managing all aspects of admissions into the CASA PROMESA Nursing Home. This role involves ensuring that all necessary documentation is completed and reviewed, maintaining census records, and conducting outreach to increase referrals. The position requires effective communication with referral agencies and the ability to manage data related to admissions and discharges.

Requirements

  • 3 - 5 years of experience in a similar role.
  • Healthcare insurance claims background.
  • Bachelor's degree preferred but not mandatory.
  • Bilingual in English and Spanish.
  • Willingness to work weekends when required.
  • Proficient in Microsoft Word, Excel, and PowerPoint.

Responsibilities

  • Coordinate referrals for admissions to the nursing home.
  • Circulate medical reports for review and approval by the Medical Director.
  • Ensure consent forms are signed and picture IDs are provided for new admissions.
  • Establish and communicate admission criteria to referring agencies and hospitals.
  • Communicate with referral agencies to obtain any missing documentation.
  • Update demographic and financial information in the computer system.
  • Provide daily census reports and ensure accurate data entry.
  • Submit monthly statistics to the NY Dept. of Health and AIDS Institute.
  • Register all admissions into the computer system.
  • Maintain databases for referrals and bed hold logs.
  • Obtain authorization for Managed Care patients.
  • Notify Nursing Home Leadership Team about concurrent review status.
  • Coordinate with Social Services and Medical Department for clinical notes and documentation.
  • Submit concurrent review packages and follow up with Utilization Management team.
  • Attend morning meetings to report on admissions data and referrals.
  • Submit daily referral reports and weekly outreach logs.
  • Attend outreach events to assist in marketing efforts for the nursing home.
  • Participate in monthly Quality Improvement Meetings and provide required metrics.
  • Analyze trends in the Admissions Department and summarize findings.
