The Ambulatory Pharmacist at Hackensack Meridian Health plays a crucial role in ensuring the safe and effective use of medications for patients. This position is responsible for dispensing prescriptions and monitoring medication therapy in accordance with all applicable regulations and standards. The pharmacist will work closely with patients, healthcare providers, and ancillary personnel to ensure optimal medication management and patient care. The role requires a commitment to compliance with state and federal regulations, as well as adherence to Medical Center policies and procedures related to drug dispensing. This includes the appropriate handling of drug products, security procedures, and the completion of necessary documentation, particularly concerning controlled substances and investigational drugs. In addition to dispensing medications, the pharmacist will review and clarify prescriptions, monitor patient drug therapy regimens for potential allergies, drug interactions, and adverse reactions. The pharmacist will also perform unit inspections to ensure proper maintenance and storage of medication supplies, including emergency drug supplies. A key aspect of this role is overseeing the activities of pharmacy technicians and couriers, ensuring that all medication practices align with departmental and organizational policies. The pharmacist will also be responsible for identifying the specific needs of the patient population served, modifying and delivering care that is tailored to those needs, and effectively communicating with patients and caregivers at their level of understanding.
