About The Position

The Medical Coordinator is responsible for assessing, facilitating, and monitoring all components of client medical services within a shelter setting. This role involves transcribing medical information, assisting with medical assessments, and ensuring compliance with relevant policies and procedures. The Medical Coordinator acts as a liaison between the medical department and contracted Primary Care Providers, while also responding to urgent medical needs and maintaining electronic health records.

Requirements

  • High School Diploma or GED.
  • Minimum of 1 year experience working with children or adolescents.
  • 6 months to 1 year experience in a medical setting.
  • Advanced typing and computer skills.
  • Bilingual in Spanish and English.
  • Ability to read and interpret medical reports and legal documents.
  • Cleared tuberculosis test results and immunization documentation.
  • Cleared background check and drug test results.
  • Valid Driver's License.
  • Willingness to work a flexible schedule and be on-call.

Nice To Haves

  • Multilingual in Turkish, Arabic, Punjabi, Hindi, Bengali, Portuguese.
  • Certification as a Medical Assistant, Nursing Assistant, Phlebotomist, or related field.
  • Associate or Bachelor's degree.
  • Cultural competence and sensitivity to diverse populations.
  • Experience in community-based programs or direct experience with Unaccompanied Minors.

Responsibilities

  • Assess and monitor client medical services.
  • Transcribe medical information and assist with medical assessments.
  • Support Lead and Assistant Lead Medical Coordinators and Regional RN.
  • Develop reports and act as a liaison between SWK medical department and contracted PCPs.
  • Ensure compliance with ORR, state licensing, and SWK policies.
  • Maintain SWK's electronic health record and ORR's UAC portal.
  • Respond to clients' medical complaints and urgent needs immediately.
  • Consult with medical staff for guidance on medical needs.
  • Ensure medications are correct according to provider's orders.
  • Prepare youth for initial medical examinations and take vital signs.
  • Assist medical providers with examination procedures.
  • Dispense medication according to policy and procedure.
  • Collect and submit required reports to supervisor.
  • Assist with training new employees as directed.
  • Maintain medical inventory by reconciling daily.
  • Educate and counsel youth on medical topics as directed by PCP.
  • Schedule and coordinate medical appointments and ensure transportation.
