About the position

Under the direct supervision of the Project Director, the Community Care Coordinator provides outreach services to youth, families, and community organizations. The coordinator assists families with identifying needs, navigating community systems, and engaging in the System of Care. The coordinator leads efforts to increase community awareness of the System of Care and engage community partners in the collaborative approach.

Responsibilities

  • Provides community-based behavioral health care coordination services for individuals experiencing behavioral health concerns
  • Completes accurate assessment and individualized, family-focused, and measurable care plan with client
  • Acts as a liaison, advocate, and resource between clients, the community, and their families
  • Coordinates treatment goals with community agencies, primary care physicians, court systems, schools, and managed care
  • Provides support services, including monitoring of daily living activities, assistance maintaining benefits and entitlements, assistance with employment and education, assistance accessing resources, family support, and coordination of services and care
  • Coordinates with crisis service teams when clinically appropriate and follows up with client and family following crisis event
  • Maintains data and records in accordance with Carle Health and Trillium Place policies
  • Participates in multidisciplinary care conferences and requests consultation, where indicated, in a diligent and timely manner
  • Provides transportation to family as appropriate
  • Understands and actively promotes children's mental health system of care partnership throughout the tri-county area
  • Provides assertive community outreach and sustained efforts to engage individual clients, schools, and partner organizations

Requirements

  • Bachelor's degree
  • Practical nurse license under the Nurse Practice Act
  • Certificate of psychiatric rehabilitation from a DHS-approved program
  • High school diploma
  • 2 years' experience in providing mental health services
  • Recovery support specialist certified and in good standing with the Illinois Alcohol and Other Drug Abuse Professional Certification Association, Inc.
  • One year experience in providing mental health services
  • Occupational therapy assistant licensed under the Illinois Occupational Therapy Practice Act
  • At least one year of experience in a mental health setting
  • Minimum of 5 years supervised experience in mental health or human services
