About the position

The Wellness Medical Assistant II plays a crucial role in supporting the primary care team within the Wellness Program. This position involves providing care for patients, assisting the Nurse Wellness Educator, and contributing to a collaborative and innovative work environment. The ideal candidate is a certified medical assistant or EMT-B with a welcoming personality and a willingness to learn new skills.

Responsibilities

  • Assists in providing care for a unique panel of patients shared with Nurse Wellness Educator.
  • Conducts pre-visit planning activities for the assigned patient panel.
  • Participates in huddle with provider to prepare for daily clinic.
  • Participates in Patient Center Medical Home (PCMH) by reviewing charts for health screenings due.
  • Prepares exam room for patient visits, restocks and cleans as needed.
  • Calls patients from the waiting room to the exam room using the proper patient identifiers and pronouns.
  • Obtains the chief complaint, vital signs, allergies, surveys, screenings, consent forms (if applicable), and performs a medication reconciliation.
  • Prepares visit note and documents all of the above in Epic.
  • Collects patient glucose logs, downloads blood glucose meters and continuous glucose meters before visits and upon request from providers.
  • Provides patient education as appropriate to topic and diabetes.
  • Assists with and independently instructs patients on blood glucose meters, continuous glucose monitors, injection therapy, medications and other technology used in the care of patients with diabetes.
  • Assist with scheduling, curriculum preparation, coordination of and teaching diabetes education group classes.
  • Assist with Group Medical Visits, scheduling, coordination of group team members, prior authorization, patient intake and directing patients during groups.
  • Assist with Weight Management Program visits, scheduling, coordination of group team members, prior authorization, patient intake and directing patients during groups.
  • Collects lab samples, performs phlebotomy and EKG (if deemed competent) as needed.
  • Administers all CLIA waived lab tests and medical tests (e.g. HCG, UA, urine drug screen, ear flush, hearing and vision screenings) per clinical protocols.
  • Acts as a chaperone for medical exams as needed.
  • Administers vaccines as directed by primary care provider.
  • Coordinates in-clinic services between providers, the lab, MAs, and nurses, to assist with timely patient flow.
  • Understands and maintains standard precautions in all clinical activities, uses PPE appropriately.
  • Demonstrates teamwork and works to build cohesive, professional relationships with all CHCFC staff and patients.
  • Maintains current CPR certification.
  • Assists in emergencies as trained, call for outside help as directed.
  • Participates in quality improvement and quality assurance activities, maintains equipment in working order and cleanliness of clinic.
  • Orders lab supplies when needed.
  • Maintains equipment in good working order and notifies supervisor of broken equipment.
  • Informs person responsible for ordering supplies when stock is below PAR level.
  • Assists in weekly inventory, restocks, and rotations of stock to prevent expired stock/shortages.
  • Participates in PDSA cycles.
  • Periodically reviews the health care quality measures for the assigned patient panel and works collaboratively with the Nurse Wellness Educator to complete recommended health maintenance activities.
  • Maintains room cleaning logs.
  • Conducts CLIA waived testing controls and ensures controls are completed as required by protocol.
  • Ensures CLIA waived test supplies are adequately stocked.
  • Checks and records daily temperature logs.
  • Properly notifies supervisor if any temperature is out of range.
  • Provide phone and clerical support for general patient inquiries and tasks.
  • Manages the work queues for incoming Wellness Program referrals and schedules patients, including calling and scheduling initial visits, managing the waitlist, no shows, and reminder calls as needed.
  • Notifies PCP of patient progress as needed.
  • Assist with scheduling appointments for follow up care at completion of the visit, as needed.
  • Completes and faxes forms including but not limited to school physical, prior authorizations, DME orders, school and work excuse letters, and other state forms.
  • Assists with medication refills under the direction and supervision of the prescribing provider.
  • Assist with scheduling appointments for follow up care as needed.
  • Clearly documents all phone calls, interactions with patients, and attempts to contact patients in a timely manner.
  • Coordinates care as directed via phone, patient portal, and/or letters as needed.
  • Responds to tasks from team members in a timely manner.
  • Fills in for PTO and call outs as needed within Wellness Department.
  • Attends and actively participates in health center medical team meetings and other staff meetings as required.
  • Effectively communicates with patients and staff across departments.

Requirements

  • Associate Degree in Medical Assisting, certificate from an accredited medical assisting program such as CMA or CCMA, a RMA License, or EMT-B.
  • Proficiency in written and spoken English.
  • 1 year of experience as a medical assistant, phlebotomist, pharmacy tech, EMT, or CNA, or 1 year of experience as a medical assistant at CHCFC.
  • Exceptionally organized.
  • Ability to work harmoniously with ethnically, culturally, racially, and gender diverse staff and patients.
  • Ability to efficiently operate basic office equipment (multi-line phone, fax, photocopier).
  • Ability to use Microsoft Office Suite (Word, Excel, and Outlook) and Internet.
  • Understands and is familiar with basic medical terminology.

Nice-to-haves

  • Bi-lingual English/Spanish, English/Haitian Creole, or English/Russian a plus.
