About the position

The Wellness Director/LVN at Golden Pond Senior Living is responsible for providing nursing consultation and support to residents in an Assisted Living/Memory Care facility. This role involves assessing residents for appropriate care placement, overseeing medication management, and ensuring compliance with health regulations. The Wellness Director will also educate staff on regulatory changes and maintain communication with families and healthcare providers to ensure quality care for residents.

Responsibilities

• Assist residents with nursing consultation and support to maintain quality of life.
• Assess residents for appropriate level of care placement within the community.
• Oversee the medication room and medication technicians.
• Manage health requests that require skilled medical professional intervention.
• Educate staff on regulatory changes through in-service education and training.
• Conduct periodic reviews for areas of responsibility.
• Maintain residents' charts and coordinate with discharge planners.
• Communicate with physicians and meet with families regarding care concerns.
• Conduct care conferences and manage updates to care notes.
• Maintain and update Medication Administration Records (MARS) and conduct audits.
• Ensure compliance with controlled medication procedures and conduct training for medication staff.

Requirements

• Active CA LVN or RN License
• CPR & First Aid Training
• B.A. in health-related field (preferred)
• 2-3+ years of experience in a similar role
• Fluency in English, both written and verbal
• Knowledge of Title 22, Division 6 regulations for RCFE and related state and federal regulations.

Nice-to-haves

• Experience with Yardi software
• Experience in Assisted Living settings
