About the position
The Registered Dietitian Wellness Coach at Highmark Health is responsible for overseeing a panel of members with varying health statuses and clinical needs. This role involves assessing health management needs, utilizing data and analytics, and providing personalized care plans to encourage behavior changes and improve health outcomes. The coach will engage with members through various communication methods and collaborate with a multi-disciplinary team to ensure quality care and support.
Responsibilities
- Maintain knowledge of evidence-based interventions and industry standards for lifestyle coaching.
- Perform ongoing assessments of members' health management needs and identify appropriate clinical interventions.
- Create personalized care plans for members, addressing identified needs and barriers to care.
- Demonstrate clinical outcomes and cost savings based on wellness interventions.
- Document all activities in compliance with business processes and regulatory requirements.
- Participate in workgroups to improve wellness coaching services and develop new programs.
Requirements
- Master's Degree in Nutrition, Health Education, Health Promotion, Exercise Science, Public Health, Social Work/Psychology, or a related field.
- 3 years of experience as a Registered Dietitian, Exercise Physiologist, or Wellness/Lifestyle Coach in a wellness-focused environment.
