About the position

The Weekend Intake Coordinator plays a crucial role in the healthcare system by managing the processing of all incoming patient referrals. This full-time position requires the individual to work 40 hours a week, specifically on weekends, with the flexibility to choose two additional days. The primary responsibilities include inputting referrals into the system, verifying payer sources and eligibility, gathering necessary medical records, and responding to phone calls promptly. The coordinator is also responsible for preparing admission paperwork, scheduling appointments for admissions, following up on pending referrals, and processing Certification of Terminal Illness (CTI) forms. In this role, the coordinator will manage referrals throughout the entire intake process, which involves retrieving referrals from the fax server or triage calls. They will ensure that all initial CTI forms are sent to the appropriate physicians and track the receipt of these forms, working collaboratively with both internal staff and external customers to ensure compliance with state and federal regulations. The coordinator will also upload admissions documentation into the electronic medical record and process financial and demographic information through Medicare and/or Medicaid systems to determine eligibility and the correct hospice benefit period. Additionally, the coordinator will create accurate admission paperwork to send to the admission nurse or the patient or their representative. They will follow up on pending referrals, non-admits, and discharges, ensuring a smooth transition for patients. Secondary responsibilities include assisting with incoming and outbound phone calls, providing high levels of customer service and professionalism, and communicating effectively with patients, families, referral sources, and other departments to support patient care needs.

Responsibilities

  • Manages referrals throughout the entire intake process, including retrieving referrals from the fax server or triage calls.
  • Ensures that all initial certification of terminal illness (CTI) forms are sent to the appropriate physicians, tracks the receipt of forms, and ensures that forms are returned and processed in compliance with regulations.
  • Works cooperatively with the clinical department to provide continuity of information for patients and staff.
  • Uploads admissions documentation into the electronic medical record.
  • Processes financial and demographic information through Medicare and/or Medicaid systems to determine eligibility and the correct hospice benefit period, documenting information correctly in the electronic medical record.
  • Creates accurate admission paperwork to send to the admission nurse or the patient or representative.
  • Follows up on pending referrals, non-admits, and discharges.
  • Assists with incoming and outbound phone calls, handling each call with a high level of customer service and professionalism.
  • Communicates with patients, families, referral sources, and other departments to support patient care needs.

Requirements
