About the position

Adventist HealthCare Home Health is seeking a dedicated Physical Therapist (PT) to join their team in Southern Maryland. The role involves providing comprehensive physical therapy services, including initial evaluations, treatment planning, and patient management, while embracing the organization's mission of holistic healing. The position requires collaboration with patients and care teams to ensure effective rehabilitation and discharge planning.

Responsibilities

  • Perform initial examinations and evaluations to establish diagnoses and plans of care.
  • Provide and supervise therapy interventions based on evaluations and patient needs.
  • Reexamine patients as necessary to evaluate progress and modify care plans accordingly.
  • Plan for patient discharge, ensuring achievement of goals and appropriate follow-up.
  • Document all aspects of patient management and communicate with care team members.
  • Supervise and direct the activities of rehab aides and office assistants.
  • Manage patient schedules and inform management of any caseload issues.
  • Attend team meetings to communicate patient status and collaborate with other therapists.
  • Engage in professional development activities.

Requirements

  • Bachelor's, Master's, or Doctorate in Physical Therapy from an accredited program.
  • 1 year of site-specific experience preferred.
  • Active Maryland Physical Therapy License.
  • Valid driver's license with agency insurance requirements.
  • Active American Heart Association Basic Life Support (BLS) certification.
