About the position

As a Travel Licensed Practical Nurse (LPN) at Trilogy, you will contribute to the assessment of patients in close collaboration with Registered Nurses. Your responsibilities will include administering medications and observing patients for adverse reactions, performing routine lab tests and therapeutic services, and educating patients and their families about recommended disease treatment plans, including self-care post-discharge, holistic health needs, available resources, and follow-up care. You will also identify safety hazards and emergency situations to ensure resident safety and take corrective action as appropriate. This position involves regional travel, defined as travel for work assignments up to two hours from your home address, or full travel across the company's footprint in Indiana, Ohio, Michigan, Wisconsin, and Kentucky. The role requires full-time, 12-hour shifts with block scheduling for overnight stays, and mileage and meal reimbursement will be provided when applicable.

Requirements

  • Active and in good-standing LPN license.
  • At least one year's experience as a Licensed Practical Nurse preferred.
  • At least 18 years of age with a valid driver's license and reliable transportation.
  • Availability to work every other weekend and holiday.
